I just went through this with my sweet Miko. When you are in the throws of the emergency, you are so focused on making sure they get better, that you are not paying attention to the "big picture". Our internist at VRCC was gentle with us bringing up end of life decisions and it was absolutely the right thing to do for our little girl. ♥

I found this today and think it is very helpful and wanted to share.

Knowing the Right Time to Say Goodbye to a Pet
End-of-life decisions for animals are difficult. A veterinarian has developed a scale to help clear up the confusion.

Is It Time? Making End of Life Decisions for Pets

Pet caregivers can use this Quality of Life Scale to determine whether to continue supportive care for an aging or sick pet or whether euthanasia is a more compassionate option. Move the slider to reflect your pet's score in each category, using a scale of 0 (very poor) to 10 (best). Your score should be based on the pet's quality of life on its own or with whatever level of supportive care works for you.

This is absolutely one of the most difficult decisions we've faced, many times, with our pups. It's heart-wrenching, even when we know it's in the dog's best interest. We're guilty of trying to extend the life of one or two of our past dogs longer than we should've. Once we realized that, and tried not to do it again, then we agonized over it thinking we were making the decision prematurely.

Thanks for posting those links. I hope it helps someone avoid the mistakes we tend to make.

We are saying goodbye to Mighty Little Scout tomorrow afternoon. With doc’s knowledge and help with some medications we bought her some good quality time and cherished every day we had. But that time has ended and though she would continue in whatever we choose for her, the past 48 hours have shown reality is just not good enough for this beloved little girl.
My heart is breaking as I write this though I know it’s the most loving thing I can do for my dear little friend .
